Southwest Airlines Ticket Donation Opportunity Form


This form is to request for Southwest Airlines Vouchers for local chapter events. 

Rolling basis with full consideration  given to chapters that submit the application at least four weeks before event date.      

OCA National will accept applications for donations from all  chapters. However, applications will be reviewed on a first-come, first-served basis and tickets will be distributed based on availability and application criteria. For full consideration, be sure to email in your application at least four weeks before your event date. 

Tickets issued by Southwest Airlines will bear the date of issue and the date of expiration. Expiration dates will not be extended, and tickers will not be refundable or redeemable for cash or credit at any time, nor will tickets be replaced if lost or stolen. Tickets valid on Southwest-operated, published, scheduled service only.

Ticket holder must be 18 years or older and legal US resident. Tickets cannot be resold to a third party and additional restrictions apply for local, state, or federal elected officials or government employees. Tickets may not be used for promotional purposes unless previously approved through Southwest Airlines. No substitutions are allowed.   Each chapter will be eligible to apply for Southwest Airlines vouchers. The distribution is under the discretion of OCA National. Priority will be given to chapters who have not been awarded within a 2 year period.
